Goddard Riverside’s Manhattan Outreach Consortium provides outreach and housing placement services to adults who are chronically street homeless.

The Per Diem Housing Outreach Specialist is an hourly, non-exempt position with work offered as shifts are available.

Essential Duties

1) Homeless Outreach/Case Management services

  • Canvass sectors of Manhattan in teams of two to find areas active with people experiencing homelessness
  • Engage individuals on the street, assess for safety and program eligibility, and provide information about resources
  • Confidential paperwork: placing individuals on a pre-caseload tracking sheet, logging bedded down sightings, completing client progress notes, and completing incident reports
  • Placement Services: referring and transporting clients to shelters and/or drop-ins
  • Communicate with clients already known to the program, assist in promoting service goals, continue client’s electronic file, and escort clients when needed
  • Assist in performing clinical assessments of clients to determine when a client should be admitted to the hospital or when 911 should be called
  • Canvass areas for evidence of homelessness and record accurate findings
  • Enter accurate and complete data into computer database after each shift
  • Prepare reports on shift activities and client statuses for day teams and case managers as requested
  • Respond to 311 calls from the community within one hour
  • Drive outreach van
  • Other duties as required

Qualifications

  • High School Diploma required
  • Bachelors degree preferred
  • Valid Driver’s License
  • Experience working with individuals with histories of mental illness, trauma, substance abuse, and chronic homelessness

Physical Requirements

  • Frequent walking and standing
  • Occasional lifting

Work Environment

  • Indoors and outdoors; all weathers
  • Schedule must be flexible and you should be available at short notice: day, evening, overnight, weekend, and holiday hours.
