Develop and implement a consistent, reliable, and sustainable empanelment process at all medical sites, consistent with organizational goals regarding access, quality and continuity of care.
Duties and Responsibilities: Develop policies and procedures for patients to be empaneled to a primary care provider, including definition of active patient, PCP assignment per provider category, part-time providers, and change of PCP. Oversee and maintain a SharePoint system with live empanelment data to be used by all medical sites. Support development and implementation of reports that enable each site to document, analyze and manage PCP patient panels, patient assignments, and panel report changes on a daily basis. Visit all medical clinics and provide training to educate staff about empanelment, following established policies, procedures and protocols. Assist sites with determining practice capacity to ensure appropriate access to assigned patient population. Support systems that empower local clinical teams to manage their own panels. Other duties as needed.
Qualifications: An undergraduate degree, masters strongly preferred. 5-6 years’ experience managing healthcare projects.
